Patient is a 78 years old male admitted for a NSTEMI. He was discharged from the hospital 2
days prior for a cardiac catheterization due to a STEMI. Patient lives alone in a group home. He
is from the area and lived here during his childhood. He moved out of state for college in his 20s
then came back in his mid adulthood. Patient was married in his late 20s with his college
sweetheart but they got divorce 10 years later. He never had kids. He came back to Binghamton
which is his home to reconnect with his friends and family but most of them are no longer here.
Pt history: Acute pancreatitis
2. Diabetes
3. Hypertension
4. Benign prostatic hyperplasia
Coronary angioplasty with stent placement
2. Transurethral resection of prostate
3. Lumbar disc surgery
Patient lives alone in a community home. No family in the area. Patient never had kids, he
was married but got divorced. Support system is his case manager and his care team.
Patient smoked for the past 50years 1pack/day and is now looking at different techniques
to replace this habit.
