It’s important for physicians and care providers to connect while coordinating patient care, especially considering new health care delivery and reimbursement models, such as value-based care. As such, the medical record should also link those professionals, such as specialists, primary care physicians (PCPs), hospitals, clinics, pharmacies, laboratories, imaging centers, and physician offices, via the EHR.
The patient is also a key constituent and stakeholder in their health care coordination. Technology should go beyond a collection of data; it should enable the interoperability of different electronic health record software systems for real time access information.
