Practice:
How often are you Charge Nurse? Quantify it: # of days per week and/or # of hours you have been charge within the last rating period? Give an example of a time you were Charge Nurse, and used your role to improve care on the unit or prevent a bad outcome.
Ethics:
Give an example of an ethical dilemma you faced as a nurse. How did you handle it? How did you use the experience to improve ethical care on the unit or of other staff? How did you educate the ARC 1 nursing team about this issue? What was the outcome after your education?

Resource:
Give an example of how you have systematically saved money for the unit.
Education:
What non-mandatory trainings or classes? Have you received any certifications? Are you enrolled in school?
Performance:
Have you done any chart reviews? How did you use the results to improve results on the unit? How did you educate the ARC 1 nursing team about this issue? What are the current stats pre- and post-intervention?
Give an example of a time you reviewed a hospital policy or SOP and used the information to improve or correct the practice of your colleagues or the unit. What was the outcome?
Have you lead any Huddle Boards? What was the topic? Who participated? What were the outcomes? Did you upload the huddle board on System Redesign’s “Project Tracker” on the SharePoint?
Collaboration:
Give an example of a time you lead a project to improve care on the unit, especially if it involved other disciplines. Who was involved? What was the outcome?
Collegiality:
How many people have you precepted/mentored? Quantify it: # of hours you have spent precepting/mentoring within the last rating period? How are they practicing now?
What in-services have you given? To how many people? Who were they? How do you know they understood your education? Did anything improve on the unit as a result?
Quality of Care
Give an example of how you have improved Nursing Quality of Care indicators on your unit (examples: Infection rates, CAUTI, CLABSI, Hand Hygiene, Falls, PRN effectiveness, Pain management, Assaults/restraints). Over what time period was your intervention? How have things improved? Hint: Stats for you unit can be found on the homepage under “nursing scorecards” and also on the VA’s SAIL Report.
Research:
Give an example of a topic you researched. How did you share this knowledge with your unit? Did the research validate or change the practice of your colleagues on the unit? Please share the references for the topic for which you completed a literature review.
Random:
Are you on any committees? How often do you attend meetings? How have you taken what you learned at the committees, and used it to improve the unit? What was the outcomes?
What are you unit champion of? Give an example of how you’ve helped the unit as the champion.
