IHP 604 Module Six Memo Guidelines and Rubric
Overview
Throughout this module, you have been learning about evaluaon methods. In this assignment, you will describe different evaluaon methods for a healthcare quality improvement iniave,
jusfy this method aer its inial launch date, and discuss how evaluaon methods for this iniave should change over me. By successfully compleng this assignment, you will gain the
knowledge and skills required to complete a poron of your project due later in this course.
For the purposes of this assignment, imagine that you are employed in the quality department of a hospital. Your hospital recently implemented an ongoing professional pracce evaluaon
(OPPE) using criteria specific to The Joint Commission medical staff standard. The Joint Commission (E-dion) standard that is applied during the OPPE is:
Introducon to Standard MS.08.01.03 Ongoing Professional Pracce Evaluaon
The ongoing professional pracce evaluaon allows the organizaon to idenfy professional pracce trends that impact on quality of care and paent safety. Such idenficaon may
require intervenon by the organized medical staff. The criteria used in the ongoing professional pracce evaluaon may include the following:
Review of operave and other clinical procedure(s) performed and their outcomes
Paern of blood and pharmaceucal usage (obtain from laboratory and pharmacy directors, respecvely)
Requests for tests and procedures
Length of stay paerns
Morbidity and mortality data
Praconer’s use of consultants
Other relevant criteria as determined by the organized medical staff
The informaon used in the ongoing professional pracce evaluaon may be acquired through the following:
Periodic chart review
Direct observaon
Monitoring of diagnosc and treatment techniques
Discussion with other individuals involved in the care of each paent. including consulng physicians, assistants at surgery, and nursing and administrave personnel
As a result of the OPPE process, it was found that two credenaled providers, two urology surgeons specifically, have been found to be deficient on the OPPE in the area of infecons: both
surgeons’ post-operave urosurgery paents have been experiencing anbioc-resistant urinary tract infecons. Not only does this impact the quality of the care per the standards, but it
can also impact the social markeng of the hospital, increase the costs of care, increase paent length of stay, and decrease reimbursement.

Therefore, your task is to recommend an evaluaon method for 3, 6, and 12 months intended to evaluate the reducon in infecon rates as part of a quality improvement iniave. Then,
you will need to jusfy your evaluaon method and why the method(s) should be employed aer the iniave launch. Lastly, you will discuss how this evaluaon method should change over
me to accurately assess the iniave over me. As you complete this assignment, reflect on the following: How do you address the urinary tract infecon rates in periods of 3, 6, and 12
months, and how will you lay out a plan for these urology surgeons? Aer all, the medical execuve commiee (MEC) of your hospital is held accountable to fulfill the OPPE requirements. If
these two surgeons connue to have post-operave paents with infecons, the MEC will determine what happens next. The MEC is commied to reducing paent harm and, specifically,
they are commied to having an acve OPPE process to drive quality improvements.
Prompt
Write a memo that is to be shared with the MEC. This memo will discuss the evaluaon methods that could be employed to address the situaon described in the scenario. Use at least two
scholarly sources to support your claims.
Specifically, you must address the following rubric criteria:
Evaluaon methods at 3, 6, and 12 months: Recommend evaluaon methods for reducing infecon rates as a quality improvement iniave at 3, 6, and 12 months.
Jusfy an evaluaon method: Jusfy an evaluaon method for healthcare quality improvement iniave at 3, 6, and 12 months aer iniave launch.
Why evaluaon methods change: Discusses how evaluaon methods for healthcare quality improvement iniaves should change over me, depending on the change of infecon
rates over me.
