1. Read Chapter 9 (assigned pages), including the Case Study on pages 301-302 of the Hill & Lynn textbook (CASE: THE SPACE SHUTTLE COLUMBIA ACCIDENT).
2. Use the chapter to answer the questions below (in blue).

Page 306 lists five areas where formal and informal aspects of an organization interact in structure, culture, and/or craft. Pages 306-315 discuss these in more detail.
Discuss 2 of these 5 areas as they apply to the NASA Case Study in terms of how the culture could have been improved to possibly prevent the disaster.
PLEASE NOTE: a) Mention the cultural issue(s) that played a role and b) how 2 of the 5 areas could address the cultural issue(s).
