Survey Evaluation and Reflection
In this course, you have applied theoretical perspectives to a real-world setting and application by designing an online instructional model to address a specific audience.
In the Part 1 application, your focus is evaluation of the instructional module you designed in Modules 1-4. You will create a survey learners can use to evaluate your module for the purpose of revision.
Reflective practice is a critical component of learning. In the Part 2 reflection, you will conduct a summative evaluation of your online instructional model. You also will offer your perspectives on a number of guiding questions.
Create a Word document for your response.
Use APA format for the title page, references page, and in-text citations. Use the part titles as subheadings.
Complete Parts 1 and 2 of the assignment in the same document.

Part 1: Survey Evaluation
Design a 10-question formative evaluation, using Survey Monkey (the free version), to generate feedback on your module for the purposes of revision. Include at least 2 open response items in which learners can provide feedback in written form.

Compose an introduction to establish context for the survey and a concluding paragraph.
In the Word or text document, submit either your survey or a link to your survey in Survey Monkey.
Part 2: Reflection on Instructional Design
Develop a 1- to 2-page paper using the questions as guides. Organize your paper with an introduction, body, and conclusion.
Use at least one peer-reviewed article to support your viewpoint.
As you reflect on the instructional design framework you chose to develop your module, complete the following:
Rate the instructional design of your module on a scale of 1 to 10, with 10 being the best. Provide a rationale for your summative rating.
What would you do differently based on this evaluation and what you have learned in this course?
Using the Loops Model, discuss how you could add creativity into your design.
