Instructions. Read the Risk Mapping article. After reading the article, assign a severity and frequency number to the activities that are identified. In addition, identify two additional hazards for each activity (a total of four additional hazards) and assign a severity a frequency number for each. Complete the risk mapping chart for each of the hazards. Also, complete a hazard identification cart like the ones for the roller coaster and petting zoo below for one sport or recreation activity of your choice and one event of your choice. You must identify a minimum of four hazards for each of the activities and events you identify. Use four separate charts.
