Discuss how you would explore this patient’s case giving a rationale for further questions you would ask, consideration of flags, evaluations you could undertake and the possible hypotheses based on the information given. Decide whether you would refer. If you do, what investigations would you like to be carried out? Discuss the rationale for your osteopathic treatment and management plan for each patient. Compare and contrast your management plans in relation to both cases.

The best way to look at each case is to imagine that the patient is sitting in front of you and that you are taking a case history from them. You have some information but there are more questions you would want to ask and you should have a justifiable reason for asking them.
The clinical reasoning is really a list of possible hypotheses given the information you have. I don’t expect you to come to a diagnosis as you don’t have all the information. The signs & symptoms that the patient describes should lead you to think of some possible MSK or pathological hypotheses.
Again, with your evaluations, think about this as a patient in front of you – how would you normally evaluate a patient?
