Background: In this assignment, you will embark upon the first major step in planning a community-based health promotion program: the community health needs assessment!
Before you begin this assignment, you should choose a community that will serve as the focus of the program you will create in the assignments throughout the course. Most students choose their local community, however, you can choose any community in which you have an interest – you will learn a lot about this community over the term! Communities can be defined in a number of ways, but for this assignment, please choose a community that is defined by geography, in other words, a town, city, perhaps even a county. The best choice will be different for each student, so please reach out to your instructor if you want to talk about how to choose a community.
To help orient you to the process of community-health needs assessment (CHNA) and familiarize you with results, you will locate a CHNA that has been performed for the community you chose (or a nearby area). CHNAs can be located through the internet. Try searching for “community-health needs assessment [name of your community/county].” If you have difficulty locating a CHNA, please reach out to your instructor. You will use this CHNA as one source to inform your assignment response. You should consult other sources of data to complete the assignment including but not limited to the websites listed in Units 1 and 2, such as the U.S. Census Bureau.
Instructions:
Your Assignment should be approximately 3–5 pages in length and be written in essay format. Each of the sections listed below (except the summarizing conclusion) should be supported by relevant, highly credible* sources. Include appropriate supporting citations and references in APA format.
*Highly credible sources include: course materials, governmental agency websites, nonprofit and philanthropic organization websites, textbooks, scholarly sources.
Highly credible sources do not include: commercialized websites, blogs, popular media magazines and websites.
Please consult your instructor if the suitability of a source is in question.

Briefly describe how this assessment was conducted including the types of data collected.
Briefly describe some of the health needs noted in the CHNA.
Compare the process used to conduct the CHNA you reviewed to one commonly used needs assessment model (choose the model from this list: Planned Approach to Community Health (PATCH); Assessment Protocol for Excellence in Public Health (APEXPH); PRECEDE/PROCEED; Mobilizing for Action through Planning and Partnerships (MAPP).
