Background: In your previous assignments, you addressed several steps of the general planning process in developing a community-based health promotion program. Specifically, you assessed the health needs of the community, chose a health focus and a target population to serve, and developed the program mission, goals, and objectives. The next steps in the planning process is securing program resources, including: identifying funding, considering an appropriate budget, and designating administrative roles.

 

Instructions:

 

To complete this Assignment, you will conduct a web search to identify a grant-funding source for your program. You may select a government or private grant. An appropriate funding opportunity is grant funding for health promotion programming, not research, business development, health service administration, or other activities. When reviewing potential funding sources, look for alignment between the mission or goals of the funding source and your program.

 

Your Assignment should be approximately 2–4 pages in length, written in essay format. Each of the sections listed below (except the summarizing conclusion) should be supported by relevant, highly credible* sources. Include appropriate supporting citations and references in APA format.

 

*Highly credible sources include: course materials, governmental agency websites, nonprofit and philanthropic organization websites, textbooks, scholarly sources.

 

Highly credible sources do not include: commercialized websites, blogs, popular media magazines and websites.

 

Please consult your instructor if the suitability of a source is in question.

 

Address the following:

 

Introduction (1 paragraph)

Briefly summarize the purpose and scope of your community-based health promotion program. Be sure to include the name of the community and the target population the program will serve.

Review the need for this program.

Choose a viable funding opportunity. Describe the following:(3–5 paragraphs)

The source of the funding, including the name of the sponsoring agency or organization.

Indicate whether the funding is governmental or private

Describe the purpose of the funding: what type of activities does it support? What health issue(s) are targeted? What population should be served?

Describe the requirements to be eligible for this funding.

Identify the total funding amount available and the maximum amount per grant

State the timeframe, or the duration, of the funding.

Include an in-text citation to the reference(s) for the funding announcement in the reference list.

Explain why your program would be a suitable candidate to receive this funding. Imagine this explanation will be viewed by the funder. Make a strong case that describes:(1–3 paragraphs)

The alignment between your program mission and the purpose of the funding,

the need the program meets and the importance of the health issue, and

the relevance of the target population being served.

Discuss the process of creating a program budget.(1–2 paragraphs)

Describe typical budget line items.

Describe costs your program may incur related to the line items noted.

Estimate of the total cost of your program and explain the estimate.

Discuss program staffing needs and responsibilities.(1–2 paragraphs)

This portion should:

 

Accurately reflect the scope and purpose of the community-based health promotion program plan developed to date

Identify staff positions needed to manage and implement the community-based health promotion program, and

Describe the anticipated responsibilities of managers and staff members.

Summarizing Conclusion (1 paragraph)
