You need the year (or years, average years) and where data came from (nation, state, county, etc) and what People are in the rates and numbers (all population ages?/Total population? certain racial or ethnic groups)? Put this information all in one sentence! Here is an example:

In 2020 (Time), 25% of all aged people (Persons) in the U.S. (Place) received (a vaccine).
