At what age did you start having seizures?  Are you current on your seizure medications? What symptoms do you have after the seizures? Do you have a family history of seizures?  Do you experience a different feeling before or after your seizure? How often do you have seizures?

1. What other assessments are necessary for this patient?

A neurologic examination, EEg to evaluate seizure activity, MRI or CT scans, assessment of the patients orientation status, evaluation of postictal symptoms, blood tests inclusing glucose levels.

1. What are some of the causes of seizures?

A possible cause could be non-compliance to prescribed medications.  Another possible cause could be a head injury or structural issue within the brain such as a tumor.  Another cause could be withdrawal from alcohol or drugs. 

1. Develop a problem list from objective and subjective data.

Headache

Weakness

1. What should be included in the plan of care?

Monitor vital signs and neurological status, administer medication as ordered by physician,  administer pain medication for headache. 

1. What other risk factors are associated with this presentation?

Possible stroke or medication non-compliance.

1. Based on the readings and the subjective and objective data, this patient most likely had what type of seizure?

Since there was no loss of consciousness, I would think this was a petit mal seizure.
