Nursing 201 Group Care Plan Project
Instructions: You are required to develop a complete care plan for the following patient. All areas need to be complete. You are required to research the patient’s chief diagnosis, medications, diagnostic studies, assessments, interventions, and rationales related to the chief diagnosis. Refer to your text Fundamentals or Medical /Surgical. Remember your Scope of Practice as a LPN! Develop a chart for the careplan. Each group is to submit a chart (Word document) and Powerpoint presentation.
Powerpoint Slides- Please develop slides for each section that you will present to the class. Maximum of 15 slides not including title and reference slides. Please use solid color slides, no pictures, or themes please. Number each slide. All group members should be listed on the title slide along with patient information. 2 pts
Chief Diagnosis- Research the pathophysiology of this chief diagnosis, what is it, what causes it, how is it treated. 1 pts.
Nursing diagnosis- Provide 5 complete nursing diagnoses.
Medications-What medications could be used to treat each medical diagnoses (include dosage and route). 1 pts.
Diagnostic studies- Which labs and studies would be obtained for the chief diagnosis. 1 pts.
Assessment- Which nursing physical assessments would you expect to perform on a patient with this chief diagnosis consider the past medical history 1pts.
Goals/ expected outcomes- Refer to your text to ensure you develop 1 goals for the each nursing diagnoses. 1pts.
Interventions- List 2 interventions for each nursing diagnoses you would perform to care for this patient, provide a rationale for each intervention. 1 pts.
Evaluation- Indicate how would you evaluate if the goals for the patient were met? What is your action for the 1 goal that was not met (of your choice) 1pts.
