Research the Washington State Health Department (links below) and Oregon Public Health (links below) web sites on notifiable conditions and reporting requirements. Create a MS Excel spreadsheet indicating the following for at least 10 reportable diseases for each state: (you
choose the diseases)
- Name of the disease
- Who is responsible for reporting it
- To Whom is the reporting made to
- How is reporting to be done
- When is reporting required by
- Civil Penalties for violations (cite the law if applicable)
Your spreadsheet should use appropriate headers, color, borders, etc to create a professional looking document.
