Describe a time when you observed a nurse interview, action or question to a patient that NEGATIVELY impacted history-taking and hurt the patient-nurse relationship  

Please list abnormal findings that you may encounter during a general survey.  For instance, if you were to consider age, sex, level of consciousness, skin color, facial features, overall appearance, status, nutritional status, body symmetry, etc. what might you see that you would want to note or report?  

-Of the four aspects of physical exam, which do you struggle with?  Hint: The skills requisite for the physical examination are: inspection, palpation, percussion, and auscultation.  Do you have a strong and weak skill?
